How Our Team Works Together to Provide Your Best Care

As a client with Kadan, you are matched with the best team member for your required level of care. Each person has a specific role in your care, and all are dedicated to your well-being.

Client Experience Manager

Your Client Experience Manager is your primary point of contact throughout your care journey. This person is assigned to your case from the beginning and serves as your advocate within Kadan. They communicate with you weekly at minimum and coordinate all aspects of your care. Please direct all questions, concerns, schedule changes and special requests through your Client Experience Manager, who will ensure the appropriate response from anyone associated with your care. 

Clinical Coordinator

Your Clinical Coordinator is a Registered Nurse who performs a comprehensive initial assessment to identify your specific needs. Within three days of choosing Kadan, your Clinical Liaison will contact you to develop your personalized care plan. They will review and update this plan every 90 days. If you need to speak with your Clinical Liaison between scheduled reviews, please inform your Client Experience Manager, who will coordinate this for you. 

Caregiver

Your Caregiver is the heart of your daily care experience. We carefully match caregivers with clients based on compatible interests, personalities, and needs. Your Caregiver provides the hands-on care outlined in your specific care plan created by the Clinical Liaison. Depending on your care schedule, you may work with multiple caregivers. For consistency of care, please communicate any care plan changes through your Client Experience Manager rather than directly to caregivers, as this ensures all information is properly documented and shared with the entire care team.
